Rare Factor V deficiency managed successfully during childbirth at Hyderabad hospital

A 39-week pregnant woman with the rare blood-clotting disorder Factor V deficiency was safely delivered at BirthRight by Rainbow Children's Hospital, Financial District, following coordinated multidisciplinary care, the hospital said on Tuesday.

The woman, who had received prenatal care at a rural facility, was referred to the hospital after routine blood tests at 38 weeks revealed abnormal blood coagulation.

Doctors found that she had been diagnosed with Factor V deficiency during childhood following an unexplained rash. Although she had remained symptom-free for more than two decades, the condition posed a significant risk during childbirth, particularly of severe postpartum hemorrhage.

Factor V deficiency is a rare inherited bleeding disorder in which the blood does not clot normally. There is no cure or standard daily treatment, requiring careful management during situations such as childbirth, the hospital said in a release.

Senior Consultant Obstetrician Dr. Himabindu Annamraju, a specialist in high-risk pregnancy, coordinated a multidisciplinary team comprising a clinical haematologist, transfusion medicine consultant and specialised anaesthetist.

The patient and her family were counselled about the risks, including the possibility of requiring multiple blood-product transfusions.

Cryoprecipitate, a blood product containing clotting factors, formed an important part of the treatment plan.

Blood products were administered as required to support the patient’s clotting ability during delivery.

The baby was delivered safely and the mother remained stable throughout the procedure.

She was closely monitored following delivery to manage her coagulation status and recovery from the transfusions, the hospital said.

"Factor V deficiency in pregnancy is exceptionally rare, and there is no fixed protocol to fall back on. What made the difference here was preparation — identifying the risk early, bringing every relevant specialist to the table before delivery, and having the blood products ready and available the moment they were needed," Dr Annamraju added.
