A child may require a Bone Marrow Transplant in three clinical situations:

There are three forms of stem cell transplant used in Pediatric BMT:
• Allogeneic - stem cells are taken from a human leukocyte antigen (HLA)-matched donor, who may be a related or unrelated individual
• Autologous - the child's own stem cells are collected before a conditioning regimen and reinfused after the procedure
• Syngeneic - stem cells are sourced from the child's identical twin

Stem cells for a Bone Marrow Transplant can be harvested through three methods:
• Hip bone (iliac crest) - blood is collected from the centre of the marrow space under anaesthesia
• Peripheral blood - intravenous catheters filter the bloodstream to collect circulating stem cells
• Cord blood - umbilical cord blood donated at birth and stored in a stem cell registry is used; it is rich in hematopoietic stem cells

A Bone Marrow Transplant carries several known risks that are closely monitored by the specialist team throughout the procedure:
• Organ complications - the heart, kidneys, lungs, and liver may be affected and require continuous monitoring
• Infection - a weakened immune system increases susceptibility to bacterial, fungal, and viral infections; preventive medications and Cytotoxic T Lymphocytes (CTLs) may be used
• Mucositis - chemotherapy-related inflammation of mucous membranes in the mouth, throat, and gastrointestinal tract; pain and palliative services reduce discomfort
• Graft-Versus-Host Disease (GVHD) - the immune system may react to the donor graft; managed with immunosuppressive medications
• Thrombotic Microangiopathy (TMA) - inflamed blood vessels causing clots, elevated blood pressure, and potential kidney injury
